NHS Prescription Service - Customer exempt from NHS fee
Day Lewis NHS Prescription Delivery Service allows you to have your prescription dispensed and delivered free of delivery charges, to your home or workplace, within 1 to 2 days of receipt.
Simply complete 4 easy steps to use our online NHS prescription service.
Step 1 - Check whether we can Deliver Your Medicine
We are restricted from posting controlled drugs or refrigerated medicines. Click
here for a list of medicines we can’t deliver. If we are unable to post your item your local Day Lewis store will be able to dispense it for you. Click

Step 2 – Fill in the back of your prescription form
Please make sure you clearly mark the relevant box on the back of your prescription form confirming why you don’t have to pay.
In some cases, the NHS will require proof of exemption. Please click
here to check whether you need to provide evidence. If so please enclose a signed photocopy rather than the original document.
Step 3 – Complete Your Patient Information Form
This form requests all the necessary information we need to process your prescription. You can download this form in either Adobe PDF or Microsoft Word format. Once you have opened up the form please print out a blank copy and fill in the requested fields.
Step 4 – Send us Your Signed Prescription
Please send your signed prescription, your Patient Information Form and a copy of your exemption evidence (if applicable) to:

Please allow 5 working days from the date you post your prescription for your medicines to be delivered.
You will be required to sign for the delivery.
